A set of questions with a choice of answers, devised for the purpose of a survey or to acquire additional information, so as to create a report on the answers derived from the questionnaire.

A questionnaire contains questions that must ultimately be answered by users on Self-service forms or Planon apps. The questionnaires must be linked to standard order(s) to be able to view the questions / answers in Work Orders .
